Community Support Program

 

 

Meeting the Need

 

 

     The Indiana State Department of Mental Health estimates that of the 130,124 persons currently living in the four county area of DeKalb, LaGrange, Noble and Steuben Counties served by the Northeastern Center approximately 2.3%, or 2,993, suffer from Serious Mental Illness.

 

 

     The Northeastern Center’s Community Support Program, through a team of trained Social Workers, Registered Nurses, Case Managers, and Mental Health Technicians, working under the direction of a Psychiatrist, is available to provide services to individuals and their families to facilitate coping and recovery from Serious Mental Illness.

 

 

     The Community Support Program is committed to providing a practical and realistic array of functional services to adult consumers with a history of long-term Serious Mental Illness.  Each consumer may find assistance in reaching for and achieving his/her potential in the least restrictive and most appropriate setting currently available within the constraints of limited resources.

The Program Components

 

 

     The Community Support Program is a case management program committed to providing a practical array of services to adult consumers with a history of long term serious mental illness.  The Program Components are:

 

·         Access to psychiatric evaluation/medication management, symptom monitoring and management.

 

·         Crisis intervention.

 

·         Treatment planning and treatment coordination focused on recovery.

 

·         Assistance in obtaining government entitlements.

 

·         Assistance with daily living skills (budgeting/banking; cooking/nutrition; and other needs).  

·         Problem solving/development of coping skills.

 

·         Assistance in locating and obtaining community resources, transportation and housing.

 

·        Mental health education for individuals and their families.

 

 

Northeastern Center Services

 

 

     The Community Support Program offers access to the following Northeastern Center services:

 

·         Semi-Independent Living Program - supervision to individuals living on their own and in need of additional financial assistance.

·         Supervised Group Living - transitional group homes for individuals seeking training in independent living and social integration.

·         Alternative Family for Adults - specialized residential setting offering a family atmosphere.

·         Consultation - counseling and education sessions provided by Northeastern Center therapists.

·         Inpatient Services - available when deemed appropriate by a consumer or staff.

·         Clubhouse Program - psychosocial rehabilitation services from 9:00 to 3:00, Monday through Friday, providing opportunities for personal growth and feelings of accomplishment.

·        Supported and Transitional Employment Programs - employment opportunities that benefit employers and provide work experiences and history for consumers.
